Job Description:
The LPN Clinical Coordinator (LPNCC) provides support to
clinical field staff and the Clinical Supervisor through completion
of staff assignments, scheduling, clinical support, and
coordination. The LPNCC will also be responsible for managing the
intake and process hospital and rehabilitation facility discharges,
and manage provider referrals. This is a full-time, office-based
position that reports to our Syracuse Headquarters Monday through
Friday 8am-4:30pm.Performance Responsibilities and
Standards:Provides administrative and technical support to the
Clinical Supervisor and clinical staff.Responsible for maintaining
availability of employees in scheduling system.Responsible for
supporting the clinicians, Clinical Supervisor in the initial and
ongoing scheduling of the patient visits for all disciplines
including onboarding clinicians.Completes reconciliation of
schedules for assigned team/clinicians/HHA supervisions.Coordinates
with intake staff and other departments for efficient and timely
initiation of services.Triage of telephone calls, taking verbal
orders, writing orders, and notifying appropriate staff.Document
accurately and completely as required.Track and disseminate
information to appropriate department(s) regarding patients in
facilities.Participates in departmental educational in-service and
team planning sessions.Participates in appropriate agency meetings
and quality improvement and utilization review activities of the
organization.Responsible for obtaining ongoing authorizations as
required.Participates in special projects and performs other duties
as requested.Requirement:Graduate of an accredited licensed
practical nursing program with current New York State Licensed
Practical Nurse registration.This role is required to report in
person to the Nascentia organizational office Monday-Friday
8am-4:30pm located in Syracuse, NY.Skills necessary include
excellent computer, verbal, and written communication
skills.Excellent organizational skills with the ability to
prioritize multiple tasks.Excellent customer service both internal
and external customers.Able to communicate effectively in English,
both verbally and in writing.Physical Requirements:Ability to sit
or stand at a desk 90% of the dayVisual/hearing ability sufficient
to comprehend written or verbal communicationAbility to express
self verbally and in writingAbility to use telephone and
communicate over the phoneFrequently uses a laptop computer to
enter orders and obtain product informationBloodborne Pathogens
Exposure Category III - Employee who rarely has contact with blood
