Job Description:
This PRN Nurse Practitioner role involves conducting
comprehensive home risk assessments for adult and geriatric
patients in various community settings, including homes and skilled
facilities. New graduates are welcome to apply, with a focus on
improving patient health outcomes through personalized assessments.
The position offers flexible hours, with compensation based on
completed assessments, ranging from $2,000 to $10,000 monthly.
Responsibilities include reviewing medical history, medication
adherence, social factors, collaborating with healthcare teams, and
providing follow-up care. Requirements include a Master--------s or
equivalent experience, active RN and NP licensure, board
certification, and at least 1 year of NP experience. Travel is
required to meet patients in their homes, with a valid
driver--------s license and insurance necessary. Benefits include a
