*Job Overview*We are seeking a Clinical Pharmacist to join our dynamic healthcare team. The ideal candidate will have a strong background in clinical pharmacy and a passion for providing exceptional patient care in a hospital setting.*#PN1**Responsibilities** Participate in interdisciplinary patient care rounds to optimize clinical outcomes.* Compare patients medication histories with inpatient orders to ensure accuracy and continuity of care.* Evaluate and manage medication regimens in accordance with hospital policies and evidence-based guidelines (e.g., antimicrobial stewardship, renal dosing, serum drug level monitoring).* Review all medication orders for appropriate dose, duration, frequency, indication, and identify potential drug interactions or contraindications.* Collaborate with physicians and care teams to resolve medication-related discrepancies and document interventions.* Serve as a drug information resource for clinical staff and healthcare providers.* Prepare sterile intravenous (IV) medications in compliance with USP and USP standards.* Ensure compliance with state and federal regulations, Joint Commission standards, and CMS conditions of participation.* Mentor and oversee pharmacy technician staff to promote safe and accurate medication preparation and dispensing.*Qualifications:** Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.* Valid New York State Pharmacist License (or eligibility for licensure).* 12 years of hospital or clinical pharmacy experience preferred, but not required.*Benefits: ** 401k with match* Eligible for full benefits 1 st of the month following 30 days of employment* 24 days of PTO in first year* Long term disability and life insurance availableJob Type: Full-timePay: $50.58 - $58.64 per hourExpected hours: 40 per weekBenefits:* 401(k)* Health insurance* Referral programSchedule:* 8 hour shift* Day shift* Evening shift* Night shiftWork Location: In personby Jobble
