Job Title: Pediatric RN - Home Care Profession: Registered Nurse Specialty: Case Manager RN Duration: 13 Weeks Shift: Weekends Hours per Shift: 8.0 Experience: One year of experience as a clinical pediatric nurse preferred, home care experience preferred License: Client Licensed RN Certifications: BLS Must-Have: Certified Home Health Care Experience Required; EPIC or Netsmart Required Description: Pediatric RN - Home Care CHHA/Oasis. Working 32 hours per week, Friday to Sunday from 8:00 AM to 4:30 PM. Training provided for the first two weeks of the contract. Responsibilities include developing care plans appropriate to the patients’ and families’ healthcare needs in collaboration with the patient, family, physician, clinical operations team, and third-party payers. Obtain medical orders and approval of medical services. Complete the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient. Ensure the completion of all appropriate clinical records needed for compliance with state and federal legislation. Deliver seamless service within established time frames. Refer to appropriate healthcare systems and community agencies/resources. Communicate with healthcare team members regarding assessment and reassessment outcomes to ensure appropriate follow-up based on patient/family care plans. Display an exemplary level of patience, courtesy, and flexibility. Interact with patients and staff in a manner that fosters positive relationships and meets the agency's goals. Ensure problem resolution at the point of contact. Maintain appropriate productivity requirements. Adhere to professional home visit performance standards. Qualifications include: Client Licensed RN and BLS certification. Strong interviewing, assessment, and communication skills required. Technical proficiency in Microsoft Office Suite, electronic medical record systems, office equipment, and multi-line phone systems. Must have a valid driver’s license and current auto insurance. Travel throughout the service area year-round is required. Skills needed include critical thinking, ability to follow standards/procedures, and make appropriate clinical judgments. Ability to build and maintain professional relationships. Must be capable of working independently with minimal supervision, adapting to change, and managing a variety of assignments.
