Job Title: Level 2 Registered Nurse Case Manager Profession:
Registered Nurse Specialty: Case Manager RN Duration: 13 weeks
Shift: Monday to Friday Hours per Shift: 8 hours (Day shift 8:00 am
- 4:30 pm) Experience: Minimum of one year of experience as a
clinical nurse in a hospital required. Nine months of experience in
home care preferred. License: RN License (New York License ONLY)
required. Certifications: BLS certification required. Must-Have:
Certified Home Health Care Experience Oasis Experience Telemetry
Experience EPIC or Netsmart experience preferred. Description: The
Level 2 Registered Nurse Case Manager is responsible for developing
and revising treatment and discharge plans in conjunction with the
patient, family, physician, care delivery team, and third-party
payors. This includes obtaining medical orders and approval of
medical services. The role involves completing and submitting OASIS
assessments, reassessments, transfers, resumptions of care,
discharges, and significant changes in condition within defined
time frames. The RN will process orders, notify physicians of
patient needs and changes in condition, and complete
certification/recertification orders and discharge summaries.
Maintaining an environment that promotes respect for patients,
their privacy, confidentiality, and property is essential. The RN
will initiate appropriate preventive and rehabilitative strategies
and provide specialized nursing services. Consultation and
education of patients, families, and other team members regarding
disease processes, self-care techniques, and preventive strategies
is also required. Supervision of LPNs to ensure patient health,
safety, and compliance with the plan of care is a key
responsibility. The RN will direct, coordinate, evaluate, and
supervise the quality of patient care services provided by the home
health agency. Interactions with patients, physicians, referral
sources, and others should foster continued positive relationships.
Supporting care management philosophy, including collaborative
customer-focused planning and case management, is essential to meet
individual health and service needs while promoting quality and
cost-effective outcomes. The RN assures completion of all necessary
clinical records for compliance with state and federal legislation
and the delivery of seamless service within established timeframes.
Maintaining current knowledge of available community-based services
is important. Flexibility and willingness to visit patients based
on need rather than geography are required. The RN ensures problem
resolution at the point of contact and verifies that authorization
and release of information forms have been signed by the patient or
responsible party. Participation in patient care conferences for
patients assigned to care is expected. The RN will monitor results
and progress while taking corrective action on quality indicators,
including customer satisfaction. Understanding the importance of
the RN function and its impact on patients, other care providers,
readmission, productivity, utilization, revenue, and expense is
necessary. Efforts to reduce or eliminate avoidable costs and
errors should be pursued actively. Quality management review
findings/audits will need prompt responses and resolution of
potential problem areas. Active participation in peer review and
quality assessment and performance improvement activities is
expected, along with maintaining productivity requirements.
Ensuring appropriate visit authorizations are in place and that
visit documentation reflects medical necessity for care in the home
setting is part of the role. Promoting an environment of
