Home Health Nursing Job Details Location: Webster, NY (Finger Lakes Region) – New York home health jobs Setting: Certified Home Health Agency Employment Type: Travel Contract (Travel RN Case Manager) Start Date: January 5, 2026 End Date: April 4, 2026 Duration: 13 weeks Schedule: Monday – Friday, 8:00am – 4:30pm (8-hour day shifts); weekends as needed Hours per Week: 40 Estimated Weekly Salary: $2,370 - $2,430 Registered Nurse Case Manager Qualifications Active RN license in New York State (NY RN license required) Minimum 1 year of clinical nursing experience in a hospital or acute care setting At least 9 months of home health care or home care nursing experience preferred Certified Home Health Care experience required OASIS documentation experience required Telemetry nursing experience required BLS certification (any provider accepted) EPIC or Netsmart EMR experience preferred Current NYS driver’s license Strong clinical assessment, critical thinking, interviewing, and communication skills Ability to work independently, exercise initiative, and adapt to changing assignments RN Case Manager Responsibilities Develop and revise individualized treatment and discharge plans in collaboration with patients, families, physicians, and interdisciplinary care teams Complete and submit OASIS assessments, transfers, resumptions of care, discharges, and significant change in condition documentation within agency timeframes Process physician orders, notify providers of patient needs and changes, and complete certification/recertification orders and discharge summaries Deliver specialized nursing care, preventive and rehabilitative strategies, and patient/family education in the home health setting Supervise LPNs and coordinate the quality of patient care services Maintain compliance with New York State and federal regulations and agency policies Participate in patient care conferences, peer review, and quality improvement initiatives Promote a respectful, confidential, and supportive environment for patients and staff Demonstrate flexibility in patient visits and ensure timely problem resolution Support care management philosophy focused on collaborative, patient-centered planning Apply now to join our home health nursing team in Webster, NY and take the next step in your travel RN career!
