Home Health RN Job Details Location: Webster, NY 14580 (Rochester area – New York healthcare jobs) Healthcare Setting: Certified Home Health Agency Employment Type: Travel Nursing Contract Assignment Duration: 13 weeks (Start: January 5, 2026 – End: April 4, 2026) Schedule: Monday to Friday, Day Shift (8:00am – 4:30pm); weekends as needed Shift Length: 8-hour shifts Hours per Week: 40 Estimated Weekly Salary: $2250-$2310 Registered Nurse Qualifications Active RN license in New York State (NY RN license required) Minimum 1 year of clinical nursing experience in a hospital or acute care setting At least 9 months of home health or home care nursing experience preferred Certified Home Health Care experience required Proficiency with OASIS documentation required Telemetry nursing experience required Experience with EPIC or Netsmart EMR preferred BLS certification (accepted from any provider) Valid NYS driver’s license Strong clinical assessment, critical thinking, and communication skills Ability to work independently and adapt to changing environments Home Health RN Responsibilities Develop and update patient treatment and discharge plans in collaboration with patients, families, physicians, and interdisciplinary care teams Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documentation for significant changes in condition Process physician orders, communicate patient needs and changes, and complete certification/recertification orders and discharge summaries Uphold patient privacy, confidentiality, and respect for property Initiate preventive and rehabilitative nursing strategies and deliver specialized home health services Educate patients, families, and healthcare team members on disease management, self-care, and preventive health strategies Supervise LPNs and coordinate, evaluate, and oversee the quality of patient care services Support care management philosophy and ensure clinical records meet compliance standards Stay informed about agency and community-based healthcare resources Participate in patient care conferences and quality improvement initiatives Promote professional growth, ongoing learning, and respect for workforce diversity Perform additional nursing duties as assigned Apply now to join our dedicated home health nursing team in Webster, NY and take the next step in your travel nursing career!
