Experience: Minimum of two (2) years of experience in a home health
setting within the last fifteen (15) years. License: Current RN
Licensure in New York State Certifications: BLS required before the
start date Must-Have: Recent experience conducting comprehensive
assessments of patients Description: Required licensure includes a
current RN license in New York State. Candidates must have a
minimum of two (2) years of experience in a home setting within the
last fifteen (15) years. Recent experience conducting comprehensive
assessments of patients is essential. Ability to review health
status questions and make evaluations based on criteria for
enrollment is required. Candidates should be comfortable working
with technology in the workplace. Knowledge of the theory,
principles, and practices of general professional nursing is
necessary. Understanding of state and agency laws and regulations
governing nursing practices is required. Excellent organizational,
interpersonal, written, and verbal communication skills are
essential. Ability to communicate and work effectively with
multi-generational consumers is important. Demonstrating
sensitivity to the concerns of others is required. Candidates must
perform comfortably in a fast-paced, deadline-oriented work
environment. Ability to successfully execute many complex tasks
simultaneously is needed. A strong ability to work independently is
critical. Job Responsibilities: Perform initial in-home
pre-evaluation of patients, including evaluation of health status,
strengths, care needs, and preferences. Guide the development of
individualized long-term care service plans. Review consumer
medical documentation or health referral forms relevant to the
case. Enter evaluation data into electronic evaluation forms and
transmit as required. Document concerns, conflicting information,
and issues that arise during the evaluation process. Emphasize
continuity of care to reduce fragmentation, duplication, and gaps
in treatment plans. Discuss with consumers their health care
options and supports needed. Initiate communication between the
Call Center and eligible consumers for enrollment into managed care
health plans. Enhance communication and collaboration with
interdisciplinary care team members. Collect quality review data
and required documentation to support outcome measurements.
Identify opportunities for health promotion and illness prevention.
Maintain working knowledge of community resources and payor
requirements. Meet all standards established for the position as
outlined in the annual performance criteria. Perform other duties
as assigned by the Regional Nurse Manager or project management. 5
