Position Summary:

The MDS LPN has the responsibility to coordinate MDS 3.0 assessments, make insurance coverage determinations, to provide MDS 3.0 education and support to staff, and to monitor and manage reimbursement rates of their assigned neighborhoods.

Qualifications & Education Required:

-- Graduation from a state accredited School of Nursing with a license and current registration to practice in New York State as a LPN

-- Three years clinical nursing experience preferably in long-term care setting. Working knowledge of MDS process required

-- Ability to interact with elders and care for their needs

-- Willingness to embrace and exemplify St. John's Brand Characteristics

-- AANAC Certification within 1 year of hire

Position Competencies:

-- Technical expertise in MDS 3.0 (Scheduling, coding, documentation, CAA, etc.)

-- Practices as an autonomous and collaborative member of the MDS & Neighborhood teams

-- Detail oriented: Prepares and submit timely and accurate MDS, Equip, and QM data

-- Educator/coach: Provides guidance to staff through interactive education

-- Decision making skills: Ability to make independent decisions regarding insurance coverage

-- Content expert in reimbursement documentation

-- Liaison: Function as a reimbursement liaison between the organization and insurance or governmental agencies

Position Responsibilities:

-- Complete nursing portion(s) of MDS for assigned units

-- Completes MDS correction paperwork as needed

-- Review & collect documentation for MDS

-- Provides coverage for any absent MDS Nurses

-- Medicare and insurance coverage management for assigned neighborhood(s)

-- Assist with scheduling of MDS assessments

-- CMI Management for assigned neighborhood(s)

-- MDS 3.0 educational resource for neighborhoods

-- Ability to maintain current MDS 3.0 knowledge through AANAC training

-- Assumes responsibility for own professional growth and development, attends in-service programs that are provided to facilitate compliance with governmental regulations

-- Help with other duties as needed that support the Core Team to fulfill St John's mission vision and values

Physical Requirements:

-- Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).

-- May be exposed to infection, odors, psycho-behavioral situations
