Job Description:
GuildCare, an innovative community-based Adult Day Health Care
Program providing services to adults with health and vision needs
is seeking an energetic and autonomous individual interested in
part-time employment working - 20 hours per week as an Outreach
Worker for their Buffalo location. - - -This individual will be
responsible for promoting the GuildCare program, seeking out
Medicaid-eligible clients in need of Adult Day Health Care services
through visits and presentations at community centers, subsidized
housing areas, healthcare agencies, etc. and obtaining appropriate
referrals. - - -A BSW or BS - -in Human Services preferred, ability
to provide own transportation for local travel and computer
proficiency required. - - Experience in outreach or marketing or
case management preferred. - - -Must be familiar with local
community services and demonstrate excellent customer service
skills . - Healthcare experience preferred. - We offer a
