Job Title: Reentry Case Manager Location: Finger Lakes Region, NY Employment Type: Full-Time, Exempt Schedule: Monday – Friday, 8:00 AM – 4:00 PM Job Summary: FLACRA is seeking a dedicated and compassionate Reentry Case Manager to support individuals transitioning from incarceration at Ontario County Jail back into the community. This role is vital in connecting individuals to housing, treatment, employment, education, and care management services. The Reentry Case Manager will also ensure that jail contract obligations are fulfilled, services are effectively coordinated, and progress is tracked and reported. Required Qualifications: Associate’s Degree in Chemical Dependency, Criminal Justice, Psychology, Counseling, or a closely related field Valid New York State Driver’s License Preferred Qualifications: Qualified Health Professional (QHP) status CASAC certification or Bachelor’s Degree in Health or Human Services Familiarity with jail-based services and community reentry programming Skills and Experience: Excellent verbal and written communication skills Demonstrated ability to work with diverse populations and service providers Strong knowledge of local behavioral health systems and community resources Ability to multi-task, manage time effectively, and remain calm in crisis Proficiency in documentation and use of electronic health records (EHR) Strong interpersonal skills and ability to work independently and as part of a team
