Employment (RIES) Employment Type: Full Time Salary: $20.50 -

something special! General Description Under general supervision,
the Ukrainian Program Case Manager provides intensive case
management to Ukrainian clients. Working in concert with agency
staff, volunteers and external partners, the case manager ensures
that cases receive required services in compliance with funder
requirements as well as CCFCS case management principles. The case
manager effectively supports ongoing and collaborative
communication amongst all program staff and is responsible for
ensuring a trauma informed care environment. Essential Duties and
Responsibilities In close coordination with resettlement program
team members ensures and documents the timely provision of all
required services per the standards and requirements outlined by
funders and other governing bodies. Prepares friends & relatives of
cases to assist in the delivery of services and material goods.
Provides airport reception services if necessary. Provides and/or
coordinates basic needs requirements – food, clothing, home
furnishings and personal items. Facilitates clients’ placement in
suitable housing. Conducts intake, orientation, and assessment of
clients. Formulates service plan and coordinates all service
provision according to requirements. Facilitates and coordinates
clients’ access to identified services providers in the fields of
health, education, public benefits, employment, and other community
resources as described in service plan. Conducts home visits and
home inspections to ensure welfare of clients as well as adequacy
of services, housing, and basic needs support. Secures payments to
vendors and clients and maintains individual client direct
assistance budgets. Documents all services in client case logs and
maintains individual case files strictly according to funder and
agency requirements. Adheres to agency case management principles,
funder service delivery philosophy, and internal program manuals.
Participates in program, department, and agency initiatives aimed
at quality improvement and routine program maintenance. Attends
regular inter-departmental and other agency staff supervisory and
informational meetings and trainings. Assists program management
and other staff in preparation of routine reports and the
completion of various program related administrative tasks.
Participates in strategic planning, program improvement work
groups, client/stakeholder satisfaction surveys, peer review and
other activities as requested by supervisor. Actively pursues
development of professional competencies related to job role
performance through reading, supervision, in-service training and
attendance at conferences and workshops. Awareness and active
support of the Agency's Mission, Vision, Values and Strategic Plan.

Education: Bachelor’s Degree in Social Work, Human Services, or
related field. Equivalent combination of education and experience
will be considered. Credentials: Valid and clean NYS Driver’s
License and use of a registered and reliable vehicle. Experience:
At least one year of experience in the Human Services field and/or
case management experience. Experience working with volunteers,
refugees, and local service providers preferred. Ability to speak
English and Ukrainian is also preferred. Physical Demands/Work
