Position Job Title: Physical Therapist Profession: Physical
Therapist Specialty: Occupational Therapist Duration: 13 weeks
Shift: 5x8.0 Hours per Shift: 8 Experience: Minimum of one year
experience in a Long Term Care/Home Care setting as Physical
Therapist. License: Current State License in Physical Therapy.
Certifications: BLS (AHA ONLY) Must-Have: - Home Health Experience
within the last year. - 1 year of PT experience in a home health
setting. - NYS PT License. Description: Provides case management
and direct patient care within a team environment. Administers
comprehensive physical therapy modalities in the home setting.
Assists the physician in evaluating the level of function and
develops the plan of care, revising it as necessary. Prepares
clinical and progress notes and advises and consults with the
family and other personnel. Participates in in-service programs.
Responsible for ensuring care delivery services are consistent with
the organization’s vision, mission, values, and strategic goals.
Promotes effective working relationships and works as part of the
team to meet goals and objectives. Utilizes resources, supplies,
and time efficiently to contain costs and increase the value of
services. Promotes positive, respectful communication to all
external sources, patients, families, and staff. Supports public
relations and marketing initiatives to the local community. Strives
for continuous improvement in physical therapy functions and care
delivery processes. Develops and revises treatment plans and
discharge plans in conjunction with patients, families, physicians,
and third-party payors. Provides an environment that promotes
respect for patients, their privacy, confidentiality, and property.
Initiates appropriate rehabilitative programs. Supervises PTA to
ensure patient health, safety, and compliance with the plan of
care. Consults with and educates patients, families, and other team
members regarding disease processes, self-care techniques, and
preventive strategies. Implements home therapy programs, including
but not limited to gait training, transfers, and activities of
daily living (ADLs). Periodically evaluates patient progress and
makes necessary adjustments to the physical therapy program.
Instructs para-professional staff in body mechanics, use of
equipment, bed mobility, transfers, gait patterns, and home
exercise programs. Directs, coordinates, evaluates, and supervises
the quality of patient care services. Interacts with patients,
physicians, referral sources, and others to maintain positive
relationships. Supports care management philosophy, focusing on
customer-focused planning and case management. Participates in
regularly scheduled patient conferences to discuss care and ensure
a comprehensive coordinated plan of treatment. Assists in the
orientation of new rehabilitation personnel as needed. Ensures
completion of all appropriate clinical records needed for
compliance with regulations and delivery of seamless service.
Maintains current knowledge of available community-based services.
Demonstrates flexibility, enthusiasm, and willingness to cooperate
while working with customers and other necessary supports. Ensures
resolution of problems at the point of contact and makes
constructive recommendations to improve quality of patient care and
operations. Maintains productivity requirements. Contributes to an
